Kiwanis Club of Kirtland
Mission Statement
To provide important information about a child in an emergency, if no adult is able to supply that information.
To incorporate a child identification program (CHAD), currently being used by the state of Ohio and 28 other states and implement it in the area covered by the Kiwanis Club of Kirtland, Ohio (Kirtland, Kirtland Hills, and Waite Hill).
The identification program (CHAD),Children Have An iDenitiy was developed as the result of a traffic accident involving a Springfield , Illinois child. Chad, a one year old at the time, was in a crash that resulted in the death of this babysitter. Although injured Chad survived the crash but no one at the scene or hospital was able to identify Chad. Luckily an emergency room technician was a cousin of Chad’s and was able to contact the family.
Obtain standardized CHAD stickers to ensure rapid identification of a child in an emergency.
· The tag will contain a place to fill out the names and phone numbers of the child’s parents, physician, and other designated emergency contact persons.
· The sticker will be of a type that is not removable and not easily smeared.

Instruction sheets will be provided for the correct location of the stickers to be placed under a child’s car seat, booster seat or on the inside of a bicycle helmet.
· The stickers should be in a standardized location for law enforcement and emergency personnel to easily look for this identification.
· The sticker shall not be in a location easily read by a casual passerby.
· Law enforcement and emergency personnel in the Kirtland area will be provided the information on the stickers and their location and encouraged to look for this identification.
· Local Hospital emergency rooms and ambulance services will be instructed on the possible availability of this information at the scene.
CHAD stickers and their instruction sheets shall be made readily available.
· Kiwanis members will distribute the stickers / instructions to the following groups along with a description of the scope of the project.
Police Departments Fire Stations Hospitals
Preschools Day Care Centers Libraries
Elementary Schools Churches Doctor Offices
Bicycle Stores Car Dealers
· Kiwanis members will also distribute stickers/ instruction sheets at local Kiwanis events, i.e. Strawberry Festival
Seek public/private partnerships to help sponsor and share printing costs. Sponsors information can be personalized and printed on the Chad sticker.
Kiwanis Club of Kirtland information shall be personalized and installed on the CHAD sticker.
Share the project and with other Kiwanis Clubs, Builders Clubs and the Key Clubs for their help in expanding the identification project.

The C.H.A.D. Program
Making sure all children have emergency identification
ü To ensure rapid identification of children in emergencies, parents and guardians of small children are encouraged to fill out the information of the child (full name, birth date, address), parents (names, home and work phone numbers), physician (name and number) and another emergency contact should the parents not be reachable (their relation to the child and phone number).
ü Place the sticker on the right front corner (child's right), under the cushion or center back of the child's safety seat. (IMPORTANT: It should not be visible to a casual passer-by).
ü CHAD stickers for bikes were created for similar reasons. Place this sticker under the bike seat or inside the shell of a helmet so your child can be quickly identified in an emergency.
ü Be sure to check every so often to make sure that the sticker is in place and legible.
